CLINICAL AND LABORATORY DIAGNOSIS OF INFECTIONS TRANSMITTED BY IXODES TICKS IN THE PERM KRAI

Abstract

The etiological structure of infections transmitted by Ixodes ticks (ITIT) was established in 467 (out of 522) patients of the Perm Regional Hospital for Infectious Diseases with the help of Laboratory Techniques Set, which included ELISA, PCR and PHOSPHAN М. There was stated that ITIT occurs most frequently - 235 (45, 0%) patients, tick borne encephalitis (TBE) -more rarely: 54 (10.4%), human granulocytic anaplasmosis (HGA) - 26 (5.0%) and human monocytic ehrlichiosis (HME) - 9 (1.7%). Various mixt-infections were detected in 143 patients (27.4%). In 55 patients (10.5%) the etiology of the disease remained to be unexplained. The clinical course of ITIT caused by genospecies B. afzelii and B. garinii is described.

THE CLINICAL EFFICIENCY OF ADAMANTANE DERIVATIVE IN THE CORRECTION OF POSTENCEPHALITIC SOMATOGENIC ASTHENIA

Abstract

An open, randomized clinical study in two groups of reconvalescents of tick-borne encephalitis suffered from postencefalitic somatogenetic asthenia. The first group was consisted of 14 patients who received adamantane derivatives - ladasten at a daily dose of 100 mg for 25 days. The second comparison group included 18 convalescents who were followed up without an prescription of Ladasten. Before and after treatment, the examination with the use of a Visual Analogue Scale fatigue, scale of subjective evaluation fatigue - Multidimensional Fatigue Inventory (MFI-20), Beck Depression Inventory, D. Spielberger's test (State-Trate-Anxiety-Inventory (STAI)), adapted for Russia by Yu. L. Khanin, a questionnaire for detection signs of autonomic changes. After receiving ladasten patients showed significant dynamics of the drop in scores accordingly to visual analogue scale fatigue by 2.5 times as opposed to those in the second group, where the fall was only by 1.4 times. In the first group there were also significant fall in scores of all scales MFI-20 (p = 0.0001), Beck Depression Scale (p = 0.001), Spielberger-Khanin test (p = 0.002) and the questionnaire for evidence of autonomic changes (p = 0.001). The obtained data suggest that Ladasten at a daily dose of 100 mg is highly effective drug with antifatigue, anxiolytic and vegetostabilizing effects and can be recommended in rehabilitation period to patients after the tick-borne encephalitis with manifestations of postencefalitic somatogenetic asthenia.

THE STUDY OF THE LEVEL OF NEUROTRANSMITTER SEROTONIN FOR THE PREDICTION OF THE PARALYTIC FORM OF TICK-BORNE ENCEPHALITIS

Abstract

This paper presents the results ofa quantitative study ofserotonin in blood platelets in patients in the acute period ofencephalitis. Regardless of the clinical form of infection, in patients, there was revealed a significant decrease in this index 203,7 ± 107,1 ng/ml relatively to control values: 585,4 ± 258,8 ng/ml, respectively (p <0.05). The analysis of the concentration ofplatelet serotonin, performed in the eruptive phase of the disease in patients with various forms of tick-borne encephalitis, allowed to set more lower index in the paralytic form (111,8 ± 40,1 ng / ml) than in non-paralytic one (237,1 ± 97,7 ng/mL, p <0.05). The presence of significant difference between severe paralytic and less severe non-paralytic form of tick-borne encephalitis in the eruptive phase of the disease makes it possible to use this index as a prognostic criterion for assessing the severity of neuroinfection.

Abstract

Currently registration of community-acquired pneumonia and purulent meningitis caused by S. rneumoniae patients in HIV-positive patientsis is not carried out, there is absent the description of the clinical symptom complex. In this regard, the purpose of the work was the detection of the features of the course of pneumococcal infection with the lesion of the respiratory and nervous systems in HIV infection. A clinical and laboratory examination of 110 HIV-positive patients with community-acquired bacterialpneumonias andpurulent meningitises was performed. Community-acquiredpneumonia fulminates with the massive damage of the lung tissue, the presence ofpleural effusion, the early development of respiratory failure, decreased SaO2<92%, frequent complications and fatal outcomes. CNS pathology ofpneumococcal etiology in HIV-positive patients is manifested by the development of purulent meningitises and meningoencephalitises having a severe progressing course and high mortality. The lesion of the respiratory and the nervous systems caused by S. pneumoniae in HIV infection is observed at the 4A and 4B stages of the disease, in the absence of a critical reduction indicators CD4 lymphocytes.

Abstract

At present, the problem of vaccination of HIV-infected patients from pneumococcal disease is still poorly studied. There is no consensus on the effectiveness of immunization, the appropriateness and timing of revaccination. The aim of the study was to examine the duration and power of post-vaccination pneumococcal immunity in HIV-infected adult patients. A prospective comparative randomized study was performed, 100 HIV-infected patients were vaccinated with a polysaccharide 23-valent pneumococcal vaccine. Comparison group was consisted of 100 cases, not vaccinated against pneumococcal infection. For detection of the power of immunity through 2 years blood serum level of IgG antibodies to a mixture ofpolysaccharides (SP) S. pneumoniae incorporated in the vaccine were determined, the comparison of the incidence of acute respiratory infections, pneumonia and bronchitis was carried out, and their etiological transcript with the help of bacteriological examination of sputum was performed. The obtained results indicate to the powerful post-vaccination immunity with the high level of seroprotection and significant preventive effect through two years after vaccination.

Abstract

A comparative evaluation of the reactogenicity and preventive efficacy of the use of Prevnar Pneumococcal 7-valent conjugate vaccine in healthy (N=95) and frequently ill children (N=30) has been performed. Under administration Vaccine was characterized by low reactogenicity and high prevention efficacy. Index of prophylactic efficacy among long-term and frequently ill children was 46.0 that’s 6 times higher than among healthy vaccinated children - 7.55.

CLINICAL AND LABORATORY FEATURES OF HHV-6 INFECTION IN IMMUNOCOMPROMISED CHILDREN FOLLOWED UP AT THE CHILDREN’S POLYCLINIC

Abstract

In a cohort of 100 immunocompromised children aged 3 months to 12 years in continuous screening of saliva and urine with a qualitative polymerase chain reaction (PCR) assay DNA herpes virus 4, 5, 6, type (CMV, EBV, HHV-6) were verified in 76% of cases. DNA herpes virus type 6 was detected significantly more often (56%). On the material of the primary medical documentation and data of clinical and laboratory examination in conditions of a children's polyclinic there was performed the analysis of clinical and epidemiological risk factors for intrauterine infection (IUI), and clinical and laboratory markers of secondary immune deficiency (SID). It was made a suggestion about predominant vertical transmission of HHV-6 during the antenatal period. Based on 100 percent of detection of opportunistic herpes viral DNA in children aged 1-3 years the tactics of "antigenic sparing" in young children with SID was justified. To optimize the diagnostic measures in respect of immunocompromised children in conditions of a children's polyclinic the performance of a comprehensive screening PCR testing for HSV infections, including HHV-6, as well as immunological and bacteriological control is proposed.

Abstract

The article presents the characteristic of laboratory test results on micromycetes of objects of the environment in the medical infectious institution. 76 samples were investigated. The high micromycetes contamination level of the hospital premises was found, positive samples accounted for 97,36%. In the etiological structure of micromycetes mold fungi prevailed. The most unfavourable premises of the hospital on micromycetes dissemination were twenty-four hour stay wards and the procedure unit.

THE CLINICAL COURSE OF INVASIVE CANDIDIASIS IN HIV-INFECTED PATIENTS

Abstract

The article presents the results of observation of 21 patients with invasive candidiasis, developed on the background of HIV infection for the period 2007-2013. The features of clinical course of the disease and the risk factors for its occurrence are described. The clinical ркЫп of invasive candidiasis in HIV-infected patients is similar to the clinical picturde seen in bacterial sepsis, it is characterized by the severe course and the high fatality. The risk factors are the pronounced immune deficiency, the absence of antiretroviral ^тру and the presence of 2 or more foci of the s^etAnal candidiasis. The diagnosis of the hematogenous dissemination should be carried out in all HIV-infected patients with the reference symptoms.

EFFECTIVENESS OF DOMESTIC ANTIRETROVIRAL NRTI CLASS DRUG IN THE MONOTHERAPY OF HIV INFECTION

Abstract

The effectiveness of domestic preparation Phosphazide was proved during the performance of the retrospective analysis of a clinical case of monotherapy in a 28-year-old female with HIV infection. Phosphazide was prescribed after the achievement of virological and immunological effect of combined schedules of antiretroviral therapy. The dynamics of CD4 lymphocytes, plasma HIV RNA [viral load test (VL)] were followed up every 3 months for 372 weeks. The persistent decline in VL and keeping it at the level less than 3,79 log10 in combination with a positive clinical effect - the lack both of symptoms of progression of HIV infection and the development of opportunistic infections, stable level of CD4-lymphocytes> 500 cells/mcl can be considered as a positive result of monotherapy with Phosphazide.

Abstract

The aim of the study was to evaluate the effectiveness of triple antiretroviral theтру, including domestic product Phosphazide in 18 HIV-infected patients during 48 weeks. There was determined the dynamics of the clinical manifestations of the disease, the parameters of viral load and CD4 lymphocytes before antiretroviral therapy (ART), and 4, 12, 24, 36 and 48 weeks after its use. The study showed high virological and sufficient immunologic efficacy of this schedule.
