Alterations in the phenotype of cells of innate immunity in patients with polytrauma in the dynamics of the use of vaccine to prevent Pseudomonas aeruginosa

The aim of the study was to investigate the influence of multivalent vaccines for the prevention and treatment of Pseudomonas aeruginosa infection in the phenotype of innate immune cells in patients with polytrauma. There were observed 60 patients with verified diagnosis ofpolytrauma. The subjects were divided into 2 subgroups by means of the simple randomization method: the patients treated with the traditional scheme and cases who additionally received the vaccine «Pseudovac». Investigation of the phenotypic composition of innate immunity cells were performed by flow cytometry on admission to hospital, on the 10th and 20th day of hospitalization. In patients with polytrauma who additionally received the vaccine «Pseudovac» in the blood there was increased the number of monocytes with CD14hiCD16+ phenotype and HLA-DR+-monocytes level retained. The number of NKT-cells (involved in immunoregulatory processes) in patients with polytrauma increased after the vaccine treatment, whereas the content of total NK-cells reduced, but while a high level of cytokine-producing cell fraction maintained. The use of the vaccine «Pseudovac» permits to compensate immunodeficiency that occurs in polytrauma due to the formation of adaptive immunity against one of the most common infections and the activation of innate immunity.

Clinical and immunological peculiarity of acute Epstein-Barr virus infection in adults

The goal - improving the quality of the diagnosis of acute infectious mononucleosis (IM) in adults on the basis of a comparative study of cytokine status in MI, acute tonsillitis and acute viral hepatitis. Materials and methods. We observed three groups of patients hospitalized in «Specialized Clinical Hospital of Infectious Diseases of the Ministry of Health of the Krasnodar Territory» in 2012-2014: 29 patients with myocardial infarction (group 1), 25 - with acute tonsillitis (group 2), 19 - with acute viral hepatitis (group 3) and the control group. The groups were matched by sex, age and severity of the disease. In the acute phase of the disease the level of cytokines IL-1a, IL-1β, RaIL-1, IL-4, INF-y in serum was studied by ELISA. Results. At the height of the infectious mononucleosis increased content of IL-1a, IL-1β and INF-y was observed. In acute viral hepatitis significant increase in IL-1β, a less pronounced increase in INF-y, and no increase in IL-1a were registered. Acute tonsillitis is characterized by no increase in IL-1a and smaller increase in INF-y, compared to infectious mononucleosis. Significant difference between infectious mononucleosis and acute tonsillitis was a considerable rise of IL-1a and INF-y in first case. Acute viral hepatitis differs from infectious mononucleosis with pronounced increase in IL-1β. Conclusion. Determined significant intergroup cytokine status differences in patients with infectious mononucleosis, acute tonsillitis, and acute viral hepatitis may be helpful as additional diagnostic criteria for well examined infections in adults.

Molecular epidemiology of vibrio cholerae - development of the algorithm for data analysis of whole genome sequencing

The algorithm of the comparative analysis of genome-sequencing data on toxigenic strains of Vibrio cholerae, obtained on different technological platforms was developed. The algorithm includes the establishment of sampling and analysis of genomes and 1929 SNP, localized in the open reading frames. The result of the analysis of genome-sequencing indicates to the presence of several waves of importation of toxigenic strains into the territory of the Russian Federation. As noted relationship strains caused the epidemic process in 1994 in Dagestan and in the Ukraine, and they are clearly discriminated from strains isolated after 2000. The strains have caused epidemic complications in Dagestan in 1994, are distributed between three different clusters, which allows to suggest the presence of at least three independent drifts in cholera in the Republic ofDagestan during the epidemic in 1994, and the presence of strains from Bangladesh and Mozambique in these clusters may indicate to the possible origin of the source of infection. So the flash in 2001 in Kazan, probably is caused by a "Nepalese" strains. Recent events in Haiti epidemic were caused by strains of "Haitian Group", which in the same period of time often were isolated in our country but have not received distribution. In our opinion, the developed algorithm can be used to develop a single unified approach to the study of the molecular epidemiology of cholera in the Russian Federation.

GIS-technologies in the advancement of epidemiological surveillance for anthrax in the Rostov region

The article describes geo-informational system (GIS) aimed at advancement of epidemiological surveillance for anthrax in the Rostov region and developed with the use of the following resources: computerized program Quantum GIS 2.2, soil maps from "Unified State Register of Soil Resources of Russia", information submitted by experts of the Federal State Healthcare Institution "Center of Hygiene and Epidemiology in the Rostov Region", data gained from the Public State Archives of the Rostov region. GIS makes it possible to reveal patterns ofspread ofanthrax among people and animals, to carry out comparative historic data analysis, to trace dynamics and a trend of incidence, to analyze the distribution of fixed problem areas according to the types of soils, landscapes, etc. Information presented in user-friendly form will be helpful for the effective and timely decision-making.

Dirofilaria repens is an emergent parasitic disease of dogs and man in Russiania. It infects humans accidentally via vectors. Occurrence D.repens in humans is most often in subcutaneous locations. Herein, we present autochtonous cases In Russiania of human dirofilariasis repens in almost all visceral organs with special attention to pulmonary dirofilariasis. Our data confirms the concept that the human being is a facultative host of Dirofilaria repens. Dirofilariasis prevalence is probably underestimated because of its non-specific clinical presentation. A national Russian prevalence study on dirofilariasis in animals and humans is called for in order to improve the management of the disease, both in veterinary and human medicine.
