Musculoskeletal involvement is the most common manifestation of this infection, while brucellosis-induced spondylitis/spondylodiscitis, mimicking degenerative diseases of the spine, often poses challenges for timely diagnosis.<italic>Objective: </italic>To demonstrate the tactical aspects of managing a patient with acute brucellosis complicated by spondylodiscitis and epiduritis, using a clinical case as an example.<italic>Material and Methods:</italic> This case report concerns a 36-year-old patient with an epidemiological history (resident of an endemic region with occupational exposure to cattle on a farm with a high incidence of brucellosis). Clinical manifestations at disease onset included febrile fever, nocturnal hyperhidrosis, and severe lumbosacral pain with radiculopathy.<italic>Results.</italic> During the prehospital and initial inpatient stages, the patient received treatment for a spinal lesion, presumably of degenerative-dystrophic origin, without significant effect. The diagnosis of "Acute brucellosis complicated by brucellotic spondylodiscitis of the lumbar spine (L5-S1) and epiduritis" was verified at the regional infectious diseases hospital based on comprehensive data and confirmed by serological testing results (IgM and IgG ELISA to Brucella spp., Wright and Heddelson reactions). Combined etiotropic therapy (doxycycline and rifampicin) partially relieved the intoxication syndrome; however, severe pain persisted. A month later, due to deteriorating condition and increasing neurological symptoms, the patient was readmitted to the neurosurgical department. The examination and MRI data allowed us to diagnose L5-S1 spondylodiscitis with epiduritis and secondary spinal stenosis. A total laminectomy at the L5-S1 level with decompression of the nerve structures was performed, and antibiotic therapy was continued, which significantly reduced pain and improved the patient's functional status.<italic>Conclusion.</italic> This clinical observation highlights the need for early exclusion of brucellosis in patients with fever of unknown origin and spinal pain in endemic regions, as well as the importance of a multidisciplinary approach (infectious disease specialist and neurosurgeon) in spondylodiscitis with neurological deficit. A unique feature of this case is the initial misdiagnosis of osteochondrosis and intervertebral disc herniation, which led to the patient being referred to the surgical and neurosurgical departments without etiotropic treatment. Timely diagnosis and combined etiotropic and surgical treatment allow for a favorable outcome and the prevention of disability.</p></abstract><trans-abstract xml:lang="ru"><p>Бруцеллез представляет актуальную проблему здравоохранения в эндемичных регионах. и Поражение опорно-двигательного аппарата является наиболее частым проявлением данной инфекции, при этом бруцеллезный спондилит / спондилодисцит, мимикрируя под дегенеративно-дистрофические заболевания позвоночника нередко вызывают затруднения для своевременной диагностики.<italic>Цель</italic> − продемонстрировать тактические особенности ведения пациента с острым бруцеллезом, осложненным спондилодисцитом и эпидуритом, на примере клинического наблюдения.<italic>Материал и методы.</italic> Представлено наблюдение за пациентом 36 лет, с эпидемиологическим анамнезом (житель эндемичного региона, профессиональный контактом со скотом в неблагополучном по бруцеллезу хозяйстве). Клинические проявления в дебюте заболевания включали фебрильную лихорадку, ночной гипергидроз и выраженный болевой синдром в пояснично-крестцовой области с радикулопатией.<italic>Результаты.</italic> На догоспитальном и первом стационарном этапах пациент получал лечение по поводу поражения позвоночника, предположительно дегенеративно-дистрофического генеза (M47.8, M51.1) без существенного эффекта. Верификация диагноза: «Острый бруцеллез, осложненный бруцеллезным спондилодисцитом поясничного отдела (L5–S1) и эпидуритом» состоялась в региональном инфекционном стационаре на основании совокупных данных и с подтверждением результатами серологических тестов (ИФА IgM и IgG к Brucella spp., реакции Райта и Хеддельсона). Проведенная комбинированная этиотропная терапия (доксициклин+рифампицин), частично купировала интоксикационный синдром, однако у пациента сохранялся выраженный болевой синдром. Спустя месяц на фоне ухудшения состояния и нарастания неврологической симптоматики пациент повторно госпитализирован в нейрохирургическое отделение. Выполненное обследование и данным МРТ позволили диагностировать спондилодисцит L5–S1 с эпидуритом и вторичным стенозом позвоночного канала. Была проведена тотальная ламинэктомия на уровне L5–S1 с декомпрессией нервных структур, а также продолжена антибактериальная терапия, после чего удалось значительно уменьшить болевой синдром и улучшить функциональное состояние пациента.<italic>Заключение. </italic>Клиническое наблюдение подчёркивает необходимость раннего исключения бруцеллёза у пациентов с лихорадкой неясного генеза и болями в позвоночнике в эндемичных регионах, а также важность мультидисциплинарного подхода (инфекционист–нейрохирург) при спондилодисците с неврологическим дефицитом. Особенностью случая является первоначальная ошибочная диагностика остеохондроза и грыжи межпозвоночного диска, из-за чего пациента направили в хирургическое и нейрохирургическое отделения без этиотропной терапии.
