Larval stages of <italic>Dirofilaria</italic> usually occur in their primary hosts, such as cats and dogs, and rarely invade incidental hosts — human. The study described a clinical case of a patient with skull base meningioma who was admitted to the neurosurgical department for elective surgical treatment. The dense nodule was palpated in the subcutaneous tissue of the frontal area of the head. During the removal of the subcutaneous node, a thin white alive worm was evacuated during the surgery. The microscopical examination verified <italic>D. repens</italic>. Human dirofilariasis is a rare disease in the Republic of Tatarstan, Russia. However, the number of dirofilariasis cases is increasing in the recent years. Thirty-seven patients with dirofilariasis were diagnosed in the Republic of Tatarstan from 2010 to 2022.</p></abstract><trans-abstract xml:lang="ru"><p>Дирофиляриоз человека — это зооноз, вызываемый тонкими нематодами <italic>Dirofilaria</italic> (<italic>D. repens, D. immitis, D. tenuis</italic> и <italic>D. ursi</italic>), принадлежащими семейству <italic>Onchocercidae</italic>. Личиночные стадии дирофилярии обычно встречаются у естественных хозяев — кошачьих и псовых и редко у случайных хозяев — человека. В данной работе приводится описание клинического наблюдения 63-летней женщины, проживающей в Республике Татарстан, которая обратилась в отделение нейрохирургии ГАУЗ «Межрегиональный клинико-диагностический центр» Казани на плановое хирургическое лечение по поводу менингиомы бугорка основной кости. При осмотре пациентки был обнаружен плотный узел в подкожной клетчатке лобной области. Во время операции по удалению подкожного узла выделился белый тонкий червь длиной 120 мм. После микроскопического исследования червь был идентифицирован как <italic>D. repens</italic>. В Республике Татарстан — регионе Российской Федерации дирофиляриоз человека является редким заболеванием. Однако в последние годы наблюдается рост заболеваемости.
